Our clinicians conduct a comprehensive assessment and/or evaluation that meets each patient’s medical needs. They communicate with the patient’s physician to develop a plan of care individualized to every patient’s needs by providing education, safety, infection prevention, falls and hospitalization prevention and managing their disease.
